Withdrawal notice form
Consumers may use this form for a Golub Voice order carrying a withdrawal right. The form is optional; another clear statement of withdrawal is sufficient.
The contract language is Turkish. English is an explanatory translation; Turkish governs a difference in meaning. Mandatory rights remain.

Notice wording
To Zorbey Torunoğlu: I notify you that I am exercising my right to withdraw from the contract for the Golub Voice service identified below.
Information to complete
Order reference; service/plan or extra-usage purchase; order/contract date; consumer’s full name; consumer’s address; email or telephone for the reply; notice date. Add the consumer’s signature only if sending the notice on paper. Electronic notice does not require a handwritten signature or an additional reason.
Sending and timing
Send the notice before the applicable withdrawal deadline and keep a copy and sending record. Do not include card numbers, security codes or unnecessary identity documents. Where service has actually begun following your service-start approval at checkout and the withdrawal exception applies, you may still use the same contact channels for termination, defective-service or other statutory requests.
